
2025 Missouri PQC Participation Stipend – Postpartum

On behalf of the Missouri Perinatal Quality Collaborative, in partnership with Missouri Hospital Association, a limited number of stipends are being offered to stakeholders in Missouri responsible for and participating in providing fourth trimester and extended postpartum care (up to 365 days post-delivery).
The 2024 Missouri PAMR report, reviewing maternal deaths from 2017 to 2021, found most pregnancy-related deaths in Missouri occurred in the year following delivery, accounting for about 80% of deaths. About half of those happened in the first six weeks postpartum or the Fourth Trimester, due to cardiovascular disease, mental health conditions, and infections. The other half of maternal deaths occurred between 43 and 365 days post-delivery, primarily due to mental health conditions, including substance use disorder, followed by cardiovascular disease. Overall, the board determined that 84% of all maternal deaths were preventable.
The available funding is intended to support ongoing or new implementation of clinical and/or social interventions or programs designed to improve care delivery through April 2026. Scopes of work should be developed to address related postpartum maternal health outcomes in Missouri with priority given to efforts addressing cardiovascular disease; mental health conditions; infections; patient, community and/or provider education and engagement; chronic disease management; care coordination; and/or transfers of care from specialist to primary care.
Proposed activities within the application should be at a “ready to deploy” stage or be in support of ongoing implementation needs. Priority will be given to relevant projects demonstrating readiness to deploy with all required application elements received by the due date. Proposals requiring strategic planning, IRB review process, etc., will be considered for a “discovery and planning” stipend.
Funding is available through the following opportunities.
A. Extended Postpartum Care Stipend: $35,000 stipend to support extended postpartum care delivery through 12 months following delivery.
B. Fourth Trimester Care Stipend: $20,000 stipend to support fourth trimester postpartum care delivery through 12 weeks following delivery. Priority will be given to proposals focused on screening, brief intervention, and referral to treatment (SBIRT) for mental health and substance use; effective stabilization of specific pregnancy induced or chronic conditions; and care coordination.
C. Discovery and Planning: $7,000 stipend to support a discovery and planning phase for an intervention, service, or program related to the objectives outlined in option A or B.

Requirements
Awardees will:
- Participate in a community of practice meeting with other stipend recipients, to be held virtually on a quarterly basis
- By December 12, 2025:
- For stipends A and B: submit early lessons learned
- For stipend C: submit draft project plan
- By March 1, 2026:
- For stipends A and B: submit project findings and evaluation
- For stipend C: submit finalized project plan and intended actions
For awarded organizations, stipends will be disbursed over two payments. The initial payment will be made within 60 days of stipend contract signature. Organizations must complete the required deliverables and provide sufficient documentation by March 1, 2026, to MHA/MO PQC to receive the remaining payment by March 31, 2026.
